The Case of the Missing VT Isthmus

Abstract

A 28-year-old woman suffered a myocardial infarction after suffering a coronary dissection during pregnancy. She presented 13 years later with incessant ventricular tachycardia (VT) and multiple defibrillator shocks. Activation mapping revealed diffusely early activation on the left ventricular septum, and careful entrainment during VT mapping did not reveal any isthmus sites. Additional steps to find this elusive VT isthmus are discussed.
