Use and abuse of systemic corticosteroid therapy

Abstract

The corticosteroid or glucocorticoid literature is characterized by controversy and unanswered questions. This summary attempts to interpret some of that controversy and distill it into a rational approach to the systemic use of corticosteroids. The author reviews five aspects of corticosteroid pharmacology and administration: (1) the function and control of the hypothalamic-pituitary-adrenal (HPA) axis, (2) the worth of the single-dose and alternate-day therapeutic programs, (3) some complications associated with corticosteroid usage, (4) the use of injectable corticosteroids, and (5) problems with corticosteroid withdrawal.
